Health Home Policy + Procedure: Quality Management

NYC Health + Hospitals Lead Health Home (NYCHH HH) is committed to providing an array of training and development programs that will enable Network Partner Care Management Agencies (CMAs) to improve the quality of services provided, gain an understanding of evidenced based practices and standardize the use of tools and workflows, while providing person-centered care to all members.

This installment of the Policy + Procedure Webinar Series will look at the NYC Health + Hospital’s Lead Health Home Quality Management Program (QMP).  The QMP uses Quality Assurance (QA) and Performance Improvement (PI) methods to increase service quality for all members. The QMP includes a Quality Management Committee, an audit program, a training program, corrective action and member gaps in care.
