Health Home Policy + Procedure: Confidentiality and Health Home Forms

NYC Health + Hospitals Lead Health Home is committed to providing an array of training and development programs that will enable Network Partner Care Management Agencies (CMAs) to improve the quality of services provided, gain an understanding of evidenced based practices and standardize the use of tools and workflows, while providing person-centered care to all members.

This installment of the Policy + Procedure Webinar Series will look at confidentiality and the use of New York State Department of Health, Health Home Forms. The goal is for CMAs to have a working knowledge of confidentiality and to ensure the protected health information (PHI) of our member’s is treated with care.
