Care Management Academy Introductory Webinar

The landscape of health care delivery is changing dramatically which requires a workforce equipped with a thorough understanding of their scope of practice, sharp analytic skills, and who deliver care with confidence and compassion. Whether the call is to treat people in a clinical setting, their home, or a homeless shelter – leveraging motivational interviewing to teach chronic disease self-management strategies, screening to identify the resources necessary to reduce patient barriers, practicing interdisciplinary team-based collaboration, improving the patient experience – the time has come for forward thinking training opportunities be made available.  The Care Management Academy was designed with this in mind.  Over the course of 8 or 10 weeks, participants of the Care Management Academy will be exposed to a high quality blended learning curriculum that incorporates sophisticated e-learning content and action-oriented in-person classes.

Upon completion of this blended program, participants will have learned and demonstrated effective strategies that promote patient engagement, improve patient outcomes and enhance the overall patient experience.

Week 1 – January 17: Care Coordination Fundamentals

Week 2 – January 24: Effective Communication Strategies in an Interdisciplinary Team

Week 3 – January 31: Motivational Interviewing

Week 4 – February 7: Elements of Health Care Delivery

Week 5 – February 14: The Role of Care Coordination in Managing Mental Health Conditions

Week 6 – February 21: Health Coaching and Electronic Medical Records

Week 7 – February 28: The Role of Care Coordination in Chronic Disease Management

Week 8 – March 6: Crisis Management and Stress Management

After successful completion of all eight weeks of the Care Management Academy, participants will be eligible to apply for a comprehensive and robust train-the-trainer opportunity. The train-the-trainer curriculum will be delivered in two full-day trainings on March 13th and March 20th. The train-the-trainer opportunity will enhance sustainability for participating sites/organizations.
