The “Updates in Population Health at NYC Health + Hospitals” webinar presented by: Dave A. Chokshi, MD MSC live on February 24, 2020, aims to equip participants with a baseline understanding of the 1) definition of population health in the context of a large integrated health system, and 2) delineate the major components of a population health strategy.

Dave A. Chokshi, MD, MSc, FACP is Chief Population Health Officer of Health and New York City Health + Hospitals — the largest public health care system in the U.S. He also serves as Chief Executive Officer of the H+H Accountable Care Organization. Dr. Chokshi’s duties include leading a team dedicated to health system improvement, spanning innovative care models, population health analytics, primary care transformation, disease prevention, social determinants of health, care management, and implementation research. His team has been recognized with honors from America’s Essential Hospitals, the Milbank Memorial Fund, AcademyHealth, Crain’s New York, Heritage Provider Network, and VIP Community Services. Dr. Chokshi practices primary care (internal medicine) at Bellevue Hospital and is a Clinical Associate Professor of Population Health and Medicine at the NYU School of Medicine.

Target Audience

This session is intended for all Primary Care team members (MD, RD, DO, PA, NP, RN, SW). Continuing nursing and medical education credits will be provided to participants as applicable.
