The “Population Health at NYC Health + Hospitals” webinar presented by: Dave A. Chokshi, MD MSC live on January 22nd, 2018, aims to equip participants with a baseline understanding of the 1) principles and approach to implementing population health management, 2) essential elements of a comprehensive population health strategy, and 3) steps needed to incorporate it all into clinical practice.
In addition, Dr. Chokshi broadens and strengthens his talk with reviewing for our participants the importance of improving accessibility to ambulatory care – and particularly primary care, how to augment care management capacity, how to incorporate interventions that address patients’ social needs, and recommendations for building nimble and robust data analytics platforms.
Dave A. Chokshi, MD, MSc, FACP is Chief Population Health Officer of Health and New York City Health + Hospitals — the largest public health care system in the U.S. There, his duties include leading a team dedicated to health system improvement, supervising initiatives on ambulatory care transformation, innovative care models, population health analytics, chronic disease prevention and management, and implementation research. His team was recognized with the 2017 Gage Award for Quality by America’s Essential Hospitals.
