Care Management: Home Visits

Home Visits are an important part of providing coordinated care.  They can help frontline healthcare staff with identifying and reducing barriers to patient care.  Home visits are a chance for frontline healthcare staff to understand the “whole story” of what might be preventing the patient from being healthy.

This course aims to strengthen frontline healthcare worker’s ability to provide personalized patient centered services by exploring ways to conduct comprehensive home visits.

According to the National Association for Home Care and Hospice, approximately 12 million people in the U.S. require some form of home health care.  Home care services are often needed to help patients manage chronic diseases, such as diabetes and hypertension, because it is more accessible and convenient for patients.  This course will explore how to use home visits as an opportunity to help diagnosis, treat and monitor a patient’s overall health.  Methods for understanding a patient’s environment, support systems, and the challenges that may impact a patients’ overall health and wellness are reviewed. Strategies to ensure the personal safety of staff and patients during home visits are reviewed.
