Care Management: Accessing Patient Resources

Practicing patient centered care offers the potential for frontline staff, such as patient navigators and care coordination staff, to address underlying issues that might be missed by health care providers.  These staff use knowledge of their patient population, knowledge of community resources and their ability to build connections that make real differences in the lives of their patients.  When patients are treated with respect, regardless of their circumstances, they are much more likely to trust and utilize the support and resources that are offered to them.  Frontline healthcare staff are the key to making sure patients feel respected and get the community resources they need.

This course aims to strengthen frontline healthcare worker’s ability to identify and connect patients to appropriate resources, while also exploring ways staff can build relationships with resources that are often utilized by patients.

Community resources can empower patients and address difficult issues such as poverty and lack of employment. Medical issues can often be related to underlying issues such as lack of nutritious food, affordable housing or quality education.  The importance of community resources cannot be underestimated.  This course explores the various types of resources available for clinical and social service providers, as they address the social determinants of health that a person may be facing.  Additionally, participants learn about a tool that can assist and support them in proactively addressing their patient’s needs. Participants will review how to inform patients on locating services, and what organizations and healthcare professionals can provide them with additional patient resources.
