In order to provide effective patient centered care, frontline healthcare staff must use evidenced-based approaches that promote behavior changes. Staff members are increasingly being asked to practice health coaching, an evidenced-based approach, when providing coordinated care.
This course is part one of a two-part series that aims to provide an overview of the main concepts of health coaching, while also exploring the ways frontline healthcare staff can use Health Coaching to deliver care coordination services.
According to The Journal of the American Medical Association, 50% of patients leave a doctor’s visit without understanding the medical advice given to them and in only 10% of visits is the patient involved in the decisions made. Health coaching can help to improve patient understanding and engagement. In this course, participants will discuss how health coaches can be key players in driving patient behavior change and assist patients to reach self-identified health goals. The importance of a care plan, what is included in a care plan and how a health coach assists with its implementation and follow up is reviewed. Participants will learn how health coaching helps patients gain the knowledge, skills, tools and confidence they need to become active participants in their care.

Learning Objectives:
- Understand what health coaching is and in what context a care coordinator might provide it
- Describe what a care plan is and in what context a care coordinator might provide it
- Know how to identify patient strengths and potential barriers they may face in following a care plan
- Know how to use a care plan to coordinate care: follow-up on appointments, lab tests, and medication adherence
- Step 1) Locate the course you want to take
