Care Management: Common Chronic Diseases Part 3 – Heart Disease/Stroke

Chronic diseases contribute to 75% of U.S. annual health care spending.  Patients with chronic diseases often experience devastating outcomes because of limited healthcare engagement and ineffective self-management skills.  Frontline healthcare staff members can play an important role in early diagnosis and proper disease management.  This can have a significant impact on the patient’s quality of life, as well as the cost of care.

This course is part three of a four-part series that aims to provide an overview of common chronic diseases, while also exploring the role frontline healthcare staff play in disease prevention and management.

In parts one and two of this series we learned about chronic diseases that, when not managed, can increase a person’s risk for heart disease and stroke.  Heart disease is the leading cause of death for both men and women, contributing to about 1 in every 4 deaths.  Every 40 seconds someone in the United States has a stroke, and every 4 minutes someone dies of a stroke.  In part three of the chronic disease series, heart disease and stroke are examined in detail.  An overview of the causes, risk factors, warning signs, diagnosis and treatment needed to effectively provide care and guidance to patients is provided.  Methods to assist patients with adhering to the medication regimens is explored, helping to promote patient education and enhanced self-management strategies.
